Job Description

We are one of the largest specialist mental health and learning disabilities trusts in the country, with an annual income of £320m and a workforce of some 6,700 staff operating from around 100 sites in Durham, Teesside, North Yorkshire and York and Selby. An exciting opportunity has arisen within the Sedgefield Community Team for a Senior Mental Health Nurse to join a high performing Community Mental Health Team, providing evidenced based care and treatment within a recovery framework for adults with Mental Health issues.You will need to be enthusiastic and highly motivated with experience in assessments and delivery of care for patients in a community setting. You will need to fully embrace Purposeful and Productive Community Services. You will have access to high quality clinical supervision within the team and will work closely with other professionals in the multi-disciplinary team to provide care co-ordination responsibilities for a defined caseload.We are an Integrated Service and you will be required to support individuals with both their health and social care needs. This will include assessing needs in line with Care Act 2014 and implementing care packages that reflect those needs.Due to the nature of the role it is essential that applicants are able to travel independently across the locality and have access to a suitable vehicle for business purposes. If necessary, adjustments can be considered in accordance with the Equality Act 2010.
