Job Description

Duties will include:




  • Clinical Plan, undertake and evaluate a variety of fun, engaging timetable of activities for patients staying on the ward either on a one-one or group basis. The activities provided will be varied, therapeutic and should be designed to support and enhance our patient’s emotional, psychological and physical health. The activity programme that is devised should be focused around the patients abilities, likes, needs and interests and should be gender and culturally specific / sensitive

  • To monitor, document and report to the MDT the patients’ progress identifying their psychological, physical, behavioural, and social needs

  • To continuously evaluate and subsequently adapt / revise the therapeutic interventions being delivered to ensure that they are most appropriate for the patient / patients on the ward

  • To support the Dementia Nurse Specialist and be a proactive Dementia and Delirium Champion on the ward

  • Use effective communication skills to develop a rapport with the patient, family members / their carers and signpost accordingly to the MDT and / or hospital carer support advisors

  • To have an awareness of patient safety when planning all patient activities on the ward



Engaging staff:




  • To support and encourage members of the MDT (SN, HCA, Volunteers) to actively participate in the provision of activities with patients

  • To actively participate in patient board meetings and MDT meetings (where appropriate) regarding the clients care and progress



Personal development:




  • Actively participating in reviewing and reflecting on your own practice through regular clinical supervision and annual appraisals

  • Attending and completing appropriate training that is required by the apprenticeship scheme



Project / ad-hoc work:




  • To support the dementia Nurse Specialist to undertake ad-hoc projects and evaluation of service provision



Information management:




  • To ensure that all documentation (Written and electronic) is accurate and kept up to date in accordance to trust standards and local guidelines
